In owners' own words

When braking and coming to almost a complete stop, the vehicle will randomly lurch forward. This situation has caused me to run stop signs and almost hit pedestrians on crosswalks. This failure has been independently observed by myself, wife and mother. The vehicle did not display and warnings prior to the failure. The vehicle has not yet been inspected or reproduced by a third party. Service brakes
When breaking at a stop sign or light, the vehicle occasionally feels like it surges forward and I'm accelerating instead of breaking. This causes me to push harder on the brakes, which then seem to mechanically engage and then stop the van. I've had the van for a year now and it happens a few times a month, especially if I breaking on an uphill incline. Service brakes
Experienced it a number of times intermittently. When making a stop from about 25-30 mph, just before making a full stop (~10mph) while foot on the break, the vehicle lunge for a split second. The feeling of loss of braking power. At number of times, I thought I was going to rear end the veh in front of me. The last time I experienced it was last Aug 7. Service brakes
